Food Pickup by Proxy

Once you are registered with the Pantry, if you are unable to get to the pantry to pick up your food during our normal service hours, you may send someone in your place – a Proxy. If you are not able to get to the Pantry to register, please contact Kristin Jachymiak for assistance: 630.948.8136 or  kjachymiak@wscpantry.org

Your Proxy needs to bring in their own photo ID along with a signed Proxy form for the current year with their name listed as your Proxy.

Proxy forms are available at the Pantry as well as on this website. Click here for a downloadable form.
If you need help filling out the form, download this help file.
